New Student - Intake Forms 


To receive accommodations from DSPS you will need to:

STEP 1: Complete steps 1-7 of the enrollment process. Click the following links to assist you in this process:
STEP 2: View the DSPS Student Online Orientation. CLICK HERE to view.

STEP 3: Call or come into our DSPS 
offices to schedule your DSPS INTAKE counseling appointment. 

Once the Above has been completed, you will then need to complete the following:

STEP 4: Bring documentation of your disability to your INTAKE appointment along with the DSPS Intake Packet.

Below are the 3 forms you MUST also have with you at the time of your INTAKE appointment with our DSPS counselors.  If you need assistance filling out the forms or have questions, arrive 15 - 20 minutes early to your appointment and we will be more then happy to help you complete them.

  2. Student Rights & Responsibilities  
  3. Consent to Release Information

Below are options for providing disability documentation:

  • IEP (Individual Education Plan) - Must have testing results or evaluation included.
  • 504 Plan
  • Current Audiogram 
  • DD214
  • Psychological Exam/Testing 
  • Physical Exam 
  • LD Testing 
  • History from another college 
  • Eye exam from Ophthalmologist 


Please arrive at least 15 minutes early for all appointments.

Intake appointments will be 45 minutes. 
All other scheduled appointments will be 30 minutes or less.
