Are You New to the Disabled Student's Program?

Disabled Students Programs and Services is here to assist you with getting started and succeeding in your academic goals.

Be sure to bring documentation of your disability and the limitations it may cause from a professional, such as a physician or psychologist. DSPS has a form, which will communicate to this professional what the documentation is that DSPS will need. It is called a Disability Verification Form or RTF Format. Copies of IEPs and prior testing by a School Psychologist or Special Education Teacher will also be helpful.

Once completed, please return the form to the DSPS office via mail, FAX or in person and we will set an appointment with a DSPS Counselor.

If you need to request services from DSPS for your classes, then you can download, print (the following form - Request for Services Form in PDF or RTF format), and bring in, your completed form to the nearest DSPS office to you.
