Leaves of Absence

In accordance with federal and state leave laws, the Mt. San Jacinto Community College District provides eligible employees the opportunity to take unpaid, job-protected leave for certain family and medical reasons. For more detailed information, please refer to the Family Care and Medical Leave Policy. All leave of absence requests will be verified and processed through the Office of Human Resources.

Medical Certification Forms

Employees are required to obtain current medical certification from their health care provider. Please work with your physician to gather the necessary information or use one of the template forms provided below:

To request a leave of absence, or if you have any leave inquiries, please contact HR Analyst, Giselle Gil, at ggil@msjc.edu for any leave-related questions. The Office of Human Resources will review all information for completion and determine leave eligibility.

Upon review, you will be emailed regarding the status of your request for leave, which will include your leave eligibility, expected return to work date, and other important leave details, or we will let you know if additional information is required.

*IMPORTANT: If you have a need for a medical leave of absence beyond 5 days, please submit your leave request and supporting documentation.
