The following forms are intended for employee and department use only.

Catastrophic Leave Donation Form

For employees who wish to voluntarily donate leave balances to the District’s Catastrophic Leave bank

The NEOED Portal is available for current employees to update information pertaining to:

  • Direct Deposit Request

  • Employee Information Update (for address/phone/email change)

  • Tax Form Updates – Federal W-4/CA State Tax Withholding DE4

  • Name Change Request (for legal or chosen name change)

  • Emergency Contact Update – Emergency Contact/Beneficiary Designation

The NEOED Portal may be accessed via Single Sign-On Portal or the NEOED Log In webpage. Your username will be your MSJC email address. For a NEOED password re-set, please contact Human Resources.
